VACCINE REQUIREMENTS FOR STUDENTS ENTERING 7TH GRADE

The Ohio Department of Health now has two additional immunization requirements for 7th grade students. In addition to the Tdap (Tetanus, Diphtheria, and Pertussis) vaccine, students are now also required to have a Meningococcal vaccine. Meningitis is a vaccine preventable disease that is on the rise in teens and college dormitories. Your child may have already had these shots. Check with your healthcare provider for current immunization records. If your child still needs to be vaccinated, make an appointment with your healthcare provider or the Preble County General Health District Immunization Clinic. The PCGHD clinic is held every Monday (except holidays) from 9:00-11:00am and 3:00-5:00pm. Please call (937) 472-0087 to schedule an appointment.

*Documentation of receiving these vaccines will be required to begin school and avoid exclusion.

VACCINE REQUIREMENT FOR STUDENTS ENTERING 12TH GRADE

Beginning with the 2016-2017 school year, Ohio students entering the 12th grade are required to have 2 doses* of MCV4 (Meningococcal) vaccine. Meningitis is a vaccine preventable disease that is on the rise in teens and college dormitories. This means that incoming seniors will have to show proof of having had the vaccine before the beginning of school in the fall. Any student not submitting documentation may be excluded from school. To have your child vaccinated, call and schedule an appointment with your primary care physician or the Preble County General Health District Immunization Clinic. The PCGHD Immunization Clinic is held every Monday (except holidays) from 9:00-11:00am and 3:00-5:00pm. Contact the PCGHD at (937) 472-0087 to schedule an appointment or for additional information.
*If the first (1st) dose of MCV4 was administered on or after the 16th birthday, a second dose is not required.
