FinalForms is our school district’s online platform for managing student forms and athletic paperwork. It simplifies the process of collecting, updating, and tracking required documentation for students, athletes, and parents throughout the school year. All student registration forms, emergency medical forms, and athletic participation packets are completed and signed electronically via FinalForms.
How Staff Use FinalForms
As a staff member, you may use FinalForms in several ways, depending on your role:
Teachers and Counselors: You can access student emergency medical information, parent contact info, and other relevant details for field trips or in-class activities. It’s a secure way to ensure you have accurate student data.
Coaches: You are responsible for verifying that your athletes have completed all required athletic forms and physicals before they can participate in practice or competition. FinalForms allows you to check compliance in real-time, receive alerts for missing forms, and communicate directly with parents and athletes.
Administrators and Secretaries: You can monitor form completion, manage staff and student records, and ensure that all necessary documentation is up to date. The system streamlines compliance tracking for district and state requirements.
Getting Started
Login: New staff will receive an email invite to set up their FinalForms account. Follow the link to create your password.
Training: A brief tutorial or walkthrough will be provided, and help documents are available within the system.
Accessing Student Info: Once logged in, you can search for students by name and view their form status, medical alerts, and contact info.
Security and FERPA Compliance
FinalForms complies with FERPA and ensures that student data is securely stored and only accessible to authorized users. Be sure to log out when done and do not share access credentials.
