Work Permit

Below are the forms (with links) to the needed forms. 

2 forms are needed:   Work Permit Pre-application form (no proof of birth date needed)

                                    Physician’s Certificate (only needed if no current physical is on file with athletics)

Once the application is filled out by the student and the employer, students may take the form to Mrs. Hoke in the High School office for processing.

Mrs. Hoke will submit all information to The State of Ohio.  She will then need the student to sign 2 copies of the work permit.  One copy will go to the student to give to the employer and the other will go on file at the school.   Questions may be directed to Mrs. Hoke.
