You will need the following documents when registering your student:

• Birth Certificate
• Shot records
• Proof of residency (MUST BE IN PARENT/GUARDIAN'S NAME)

IF APPLICABLE, PLEASE BRING WITH YOU TO REGISTER YOUR STUDENTS

• Any custody papers or other court documents (IF NECESSARY)
• IEP/504 (IF NECESSARY)
