- Birth Certificate
- Shot records
- Proof of residency (MUST BE IN PARENT/GUARDIAN'S NAME)
IF APPLICABLE, PLEASE BRING WITH YOU TO REGISTER YOUR STUDENTS
- Any custody papers or other court documents (IF NECESSARY)
- IEP/504 (IF NECESSARY)

National Trail is a statewide Open Enrollment District. If you are interested in attending National Trail, please fill out the attached form and contact the District Office for additional information.
YOUR STUDENT WILL NOT BE ENROLLED UNTIL ALL OF THE ABOVE DOCUMENTS AND ONLINE FORMS ARE SUBMITTED AND YOU HAVE BEEN TO YOUR SCHEDULED ENROLLMENT APPOINTMENT.
